Introduction

Dyslexia Care was founded by Amanda Stokes for the purpose of carrying out dyslexia and dyscalculia assessments and tutoring. It is important to me that you are confident in the way I look after your data. This privacy policy outlines the type of personal data I may need prior to the assessment as well as how I store, handle and keep the data safe. It also states your rights and how you can contact me if you have any questions or problems. I am committed to complying with the General Data Protection Regulation (GDPR) and all other relevant data protection laws. As such, I am registered with the Information Commissioners Office and my ICO reference number is ZB887308.

I am an independent assessor who is qualified to tutor in and assess for dyslexia. 

My qualifications:

  • BEd (Hons) – Sheffield Hallam University

  • Level 7 SpLD in Specific Learning Difficlties – Leicester University

  • Member of the British Dyslexia Association (BDA)

  • Assessment Practising Certificate (APC) number: 23/APC08147

  • Level 5 Dyscalculia and Maths Learning Difficulties – Dyscalculia Association

 

What Personal Data I Collect

To provide effective dyslexia assessments, I may collect the following personal data:

  • Contact Information: Name, address, email address, phone number. 

  • Background Information: Date of birth, educational setting, any relevant background information about your learning history. 

  • Assessment Data: Information gathered during the assessment process, including test results, observations, and notes. 

  • Payment Information: If applicable, payment details for the assessment. 

  • Family Details: Details of other family members with whom a client/learner or a client/learner's school/college/workplace have given me consent to liaise.

 

 

How I Use Your Personal Data

Your personal data will be used for the following purposes:

  • To plan, conduct, and write reports on dyslexia assessments. 

  • To contact you regarding your assessment, provide updates, and answer your questions. 

  • I will share your information with your child's school, other professionals, or other third parties, if you have given me permission to do so. 

  • To comply with any legal obligations, including those related to data protection and data security. 

 

Data Security

  • I have implemented appropriate security measures to prevent unauthorized access, alteration, disclosure, or destruction of your personal data.

  • All computer systems are password protected, and assessment papers are securely stored and destroyed after a specified period.

  • I will notify you and any applicable regulator of a suspected data security breach, where I am legally required to do so. 

 

Data Retention

  • I will retain your personal data for as long as necessary to provide the services you have requested, or as required by law.

  • Assessment papers and accompanying information with responses will be shredded or deleted within six weeks of the data of the assessment.

  • The final report will be stored electronically and will be password encrypted for 6 years. 

 

Your Rights

Under GDPR, you have the following rights:

  • You can request access to the personal data I hold about you. 

  • You can request that I correct any inaccurate or incomplete personal data. 

  • You can request that I erase your personal data, subject to certain exceptions. 

  • You can request that I restrict the processing of your personal data. 

  • You can request that I provide you with your personal data in a machine-readable format. 

  • You can object to the processing of your personal data in certain circumstances. 

  • You have the right to withdraw your consent for me to hold or use your personal information. To do so, please contact me via email or post.
